i) Letter of application / Authority note from applicant (if a relative is applying on behalf of the applicant) with the following attachments:

• Photocopy of relevant pages of Passport of applicant including pages with applicant’s signature and particulars.
• Passport picture of applicant
• Address/telephone number of applicant
•  I.D. of recipient who will sign as having received the documents indicating his address and telephone numbers.
  

ii) Letter should be sent to the following address:

iii) Processing Fees:
    a) Five cedis (¢5) for local applicants
    b) Thirty dollars ($30) for foreign applicants.

Persons collecting authenticated letters should submit original copies of I.D for inspection and photocopies of I.D for the Ministry’s records.
